Privacy & FERPA Notice

The information you submit is collected voluntarily by TRIAD LLC and stored securely. By submitting this form you acknowledge the following:

What we collect: Your name, email, phone number, school, military connection type, branch of service, MOS/rate, major, expected graduation date, and a password for your account, nothing more at this stage.

How it's used: Solely to contact you about TRIAD program enrollment and to prepare your student profile for onboarding at your campus.

Academic records & FERPA: We do not access your GPA, transcripts, or any academic records through this form. A separate, explicit FERPA release will be required during formal onboarding before any academic data is ever accessed. Your rights under the Family Educational Rights and Privacy Act are fully protected.

Data sharing: Your information will not be sold or shared with any third party. It will only be accessible to authorized TRIAD program staff and, once you enroll, your campus coordinator.

Your rights: You may request deletion of your application at any time by emailing ls2ordaz@gmail.com. Submitting this form does not obligate you to enroll.

The Program

THREE PILLARS.
ONE DESIGNATION.

Complete all three pillars, maintain a 2.5 GPA, pass your mentorship review, and earn the TRIAD Triple Threat designation: a credential that follows you beyond graduation.

PILLAR 01

ACADEMIC PILLAR

Two semesters of verified academic resource engagement: tutoring, advising, writing center, and Veterans Benefits advising.

  • 3+ verified service visits, Semester 1
  • 2+ continued visits + VA benefits advising, Semester 2
  • 2.5 GPA minimum maintained throughout
  • One outreach or welcome week participation
PILLAR 02

VETERANS PILLAR

Active membership and community engagement through your campus Veterans Club, SVA chapter, or approved veteran organizations.

  • Active membership in campus vet org verified
  • 4+ community engagements logged
  • At least 1 community service or off-campus event
  • Hours tracked on your TRIAD profile
PILLAR 03

SVA PILLAR

Registered SVA membership and at least one qualifying national leadership event, connecting you to 600,000+ student veterans.

  • Registered at svavets.org
  • 1 qualifying leadership event attended
  • Leadership reflection submitted
  • Final Review Board sign-off
