The information you submit is collected voluntarily by TRIAD LLC and stored securely. By submitting this form you acknowledge the following:

What we collect: Your name, email, phone number, military connection type, and expected graduation date — nothing more at this stage.

How it's used: Solely to contact you about TRIAD program enrollment at APU and to prepare your student profile when the program launches on campus.

Academic records & FERPA: We do not access your GPA, transcripts, or any academic records through this form. A separate, explicit FERPA release will be required during formal onboarding before any academic data is ever accessed. Your rights under the Family Educational Rights and Privacy Act are fully protected.

Data sharing: Your information will not be sold or shared with any third party. It will only be accessible to authorized TRIAD program staff and, once you enroll, your campus coordinator.
